Brookhaven Health Care is Springfield’s newest premier rehabilitation and skilled nursing facility. Our newly renovated building is warm and inviting. Families, residents and visiting health care professionals all agree that Brookhaven is truly a welcome change to other facilities that they have experienced.

Brookhaven was formerly named West Park. In 2004 it was closed and remained closed for about 5 years. After a three and a half million dollar renovation, where literally every surface has been touched, we are reopened and confident that Brookhaven will become the chosen location for short term rehabilitation, skilled nursing and long term care in the Springfield area.

With our extensive remodel, many visitors are surprised that they are in a skilled nursing facility. It truly looks and has the feel of a home-like assisted living campus. Brookhaven is however a skilled nursing facility. Our focus is short-term rehabilitation and care but can and do accomodate long-term resident stays.

We are a Medicare and Medicaid certified facility. We also accept a variety of Managed Health Care plans as well as private pay and long-term care insurances. We have exceptional physical, occupational and speech therapy offered as well as restorative therapy services.
